Most of the personal information we process is provided to us directly by you for one of the following reasons:

  • You communicate with us to book an appointment
  • You complete our pre-appointment form
  • As part of the dietetic appointment

  • From a referral letter or email from another healthcare professional who has referred you to our service

How we use the personal information

We use the information that you have given us in order to:

  • Allow you to participate in the service you have chosen to do so.
  • Personal information collection is required to allow Leah Seamark, Registered Dietitian, to complete an accurate assessment, make an informed decision about the advice and support required, and to communicate this advice to you and other relevant healthcare professionals involved in your care.

How we share information

 We may share your personal data with your referrer and other healthcare professionals if you specifically request this and have consented to it.

We may be required to share personal data (for example with your GP or with the emergency services) if we consider that there is a real risk of harm to you or to others.

We will not distribute, sell or lease your personal information to third parties unless we have your explicit permission or are required by law to do so.

How we store your personal information

We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, we have put in place encrypted electronic systems and standard operating procedures to safeguard and secure the information we collect.  This includes storing electronic records and forms completed by your on Sync, which is GDPR compliant.

How long we keep personal information

If you contact Leah Seamark Diet, but do not arrange an appointment, your contact details will be kept for no longer than one year following your last correspondence or until you request for your details to be deleted.

If you have an appointment with Leah Seamark, Registered Dietitian, we are required to keep your records securely for 8 years.  We may retain your personal data for a longer period in the event of a complaint or if we reasonably believe there is a prospect of litigation in respect to our relationship with you.

For tax purposes, the law requires us to keep basic information about our customers (including identity, contact and payment information) for six years after they stop using our service.

In some circumstances, we may anonymise your personal data for research of statistical purposes in which case we may use this information indefinitely without further notice to you.

As all personal data is kept electronically, when the data is no longer required, the records will be deleted from the laptop, the recycle bin, email mailboxes (including sent mail) and any secured on-line storage systems.

Controlling your personal information

We will not distribute, sell or lease your personal information to third parties unless we have your explicit permission or are required by law to do so.

You may request details of personal information which we hold about you under the General Data Protection Regulation (GDPR). If you believe that any information we are holding on you is incorrect or incomplete, please email or write to us as soon as possible at the above address. We will promptly correct any information found to be incorrect.

What are my rights?
